i’ve known folk to joke about it but it never happend during my school medical.

A long time ago when the doctor checked new army recruits he would raise their balls and ask them to cough so he could find out of they had any problems down there.

During a check for an inguinal hernia the patient is asked to cough to increase intra-abdominal pressure that, if a hernia existed, would cause the intestine to push into the inguinal ring.
